What Does FDA Approval Mean?
FDA approval means a drug has been evaluated through controlled clinical trials and the FDA has determined that the benefits of the product outweigh the known risks for its intended use. Approved peptide drugs go through one of several regulatory pathways:
- NDA (New Drug Application): The standard pathway for new drugs, including most small-molecule and peptide therapeutics. Sponsors submit comprehensive data on safety, efficacy, manufacturing, and labeling.
- ANDA (Abbreviated New Drug Application): Used for generic versions of previously approved drugs. The applicant must demonstrate bioequivalence to the reference product.
- BLA (Biologics License Application): Required for larger biologic products, including some peptide-based therapies such as insulin analogs. BLAs require demonstration of safety, purity, and potency.
Off-Label Prescribing
Once a drug is FDA-approved, licensed clinicians may prescribe it for uses beyond its labeled indications. This practice, known as off-label prescribing, is legal and common in clinical medicine. However, off-label use is not backed by the same level of regulatory review as approved indications, and insurance coverage may differ.
Patients should discuss with their prescriber whether a peptide is being used on-label or off-label, and what evidence supports the off-label use in their specific case.
